Introduction

Raskot community hospital is a not for profit company, under Company Act 2063, which is also registered under Social welfare council. The hospital is proposed to set up a 15-bedded digitalized community hospital in Gautu, Raskot municipality, Kalikot to provide access to medical services to people of Kalikot some part of Bajura who are deprived of basic health services. Raskot, in the Karnali province – remotest province of Nepal, lies at the upper Hilly region of Kalikot. The aim is to provide a comprehensive health programme (preventive, promotive, curative, and rehabilitation), aimed at reaching target groups, by improving the access to basic health services. The project is planned to serve above 70000 people in Raskot and neighboring village who are deprived of basic health services and emergency care. The hospital will provide basic services that too in affordable cost. The locality is densely populated with marginalized communities who are poor and Dalits and particularly affected by conflict during the civil war.

Problem
  • There is absence of hospital in the area. The nearest hospital is nearly 5-6 hour journey on a vehicle from the vicinity. That too overcrowded due to only one hospital in the district
  • The nearest district hospital to Manma is 50 kilometres which is currently 15 bedded.
  • Out of 77 districts, the district itself is at last 5 in district health status of Nepal.
  • Access to affordable health care services far exceeds the income of the majority of the population Nepal. More than 50% of total population is under poverty.
  • The total district of population 1.36 lakhs (As per 2011 CBS) has only 4 doctors(2017) in district hospital.
  • Lack of any emergency care in the vicinity deteriorates the life of the locals. The poor maternal and infant mortality so lack of to emergency caesarian operations
  • Nearly 40% are illiterate in Kalikot district
  • Nearly 83% are Agriculture based (2011 CBS)
  • Life expectancy is around 58 years
  • Human development index is just 0.32
  • Around 40 km away from nearest national highway i.e. Karnali Highway
  • Major Clinical conditions that need attentions

Malnutrition (Under nutrition),

Acute gastroenteritis (AGE),

Fever of unknown origin (FUO),

Fractures and trauma,

Chronic obstructive pulmonary disease (COPD),

Upper and lower respiratory tract infections (U and RTIs),

Abdominal pain (acute abdomen, biliary colic, renal colic, acid peptic disorder, and so on),

Uterine Prolapse

Obstetric (and gynecologic) emergencies.

  • Currently only health post is serving the need of the locals which is not sufficient to treat the above clinical conditions.

Solution

A community hospital will provide and ensure affordable access to healthcare services for the poor communities in the surrounding hills of Raskot municipality. Based on vision of 15 bedded hospital of new health policy of Nepal government, hospital will:

  • provide emergency services, treatment of common diseases and injuries, maternal and peri-natal healthcare, a 24 hour/day delivery service, emergency obstetric care, child health services (including oral health services, accident prevention and rehabilitation, post trauma problems, operation services, chronic disease – diagnosis, control and referral, counselling and school health programme and preventive services) and outreach support such as seasonal camps at a community level;
  • Complete smart hospital with Digitalization, Telemedicine services.
  • be initially run by 1 doctors ( 1 MBBS), and few staff nurses (Phase 1), then a specialized doctor (MD GP), and additional nurses and health professionals in Phase 3;
  • have an minor operating theatre in phase 2 and upgradation in phase 3 to the capacity for surgery, specifically for emergency obstetrics and gynecology and other emergencies surgery; as well as a full range of simple procedures;
  • Ambulance service to carry the emergency and incapable patients from remote part of the area.
  • Provide support and outreach services to the surrounding area (Phase 3).
  • Remain as a health supporting organization for local governing system
  • Health Insurance service/Health for all
  • Exemption and free services for 10% of total patients as recommended by local bodies

Budget and Timeline

 The major portion of budget is required for setup cost of hospital building, equipment’s, Pharmacy, laboratory and digitalization of hospital.

Phase 1:  Will operate two sub phase (a) and (b)

Sub-phase (a): building a complete Out Patients’ Department (OPD),   hospital administration offices, with X ray, USG and Emergency Services. MBBS doctor will be in charge of curative care.

Sub-phase (b): addition of ward and emergency to (a).

Total cost estimated: 74 lakhs

Phase I start from the existing Ayurvedic building which cost around 100 lakhs, which will be handed over by Municipality.

Starts from 2019 January

Phase 2: Shift to own Building at Gautu
Addition of Maternity services

Total cost cumulative estimated: 167 plus

Phase 3: Addition of operating theatres, Ambulance, complete Digitalization.

MDGP doctor will take in charge of curative care.

Total cost cumulative estimated: 642 lakhs

Estimated project ends in 3 year
